Гнездная алопеция (ГA) является хроническим многофакторным заболеванием волосяных фолликулов и ногтей неизвестной этиологии с отчетливой генетической предрасположенностью и аутоиммунным патогенезом. Это частая жалоба в дерматологической практике, составляющая от 0,7 до 3,8% всех визитов. В зависимости от числа, локализации очагов и площади поражения клиническая классификация включает следующие формы заболевания: очаговую, многоочаговую, офиаз, тотальную, универсальную, сисаифо (или инверсный офиаз — ophiasis inversus), ретикулярная и диффузная. При естественном течении болезни рост волос возобновляется у 34% - 50% пациентов в течение 1 года, в то время как у 15% - 25% заболевание прогрессирует до тотальной ГА (с полным выпадением волос на волосистой части головы). Существует прямая связь между тяжестью ГА и долгосрочным прогнозом.
Доступные методы лечения могут вызвать рост волос, но не влияют на течение болезни. Потенциальные аутоиммунные механизмы, участвующие в патогенезе ГА, включают сенсибилизацию Т-лимфоцитов, в частности, CD8+ Т-клеток к фолликулярным антигенам.
Активация лимфоцитов перифолликулярного инфильтрата, характерного для ГА, вызывает высвобождение ряда цитокинов Tх1 — интерлейкина (ИЛ) -1 альфа, ИЛ-1 бета и фактора некроза опухоли (ФНО) — альфа, способных ингибировать рост волосяного фолликула и прекращать синтез волос с досрочным завершением фазы анагена.
Предлагаются разнообразные методы лечения ГА, но рандомизированных двойных плацебо-контролируемых исследований очень мало, и, как было отмечено выше, ни один вариант терапии не способен повлиять на течение заболевания.
Метотрексат (MTX) является иммунодепрессантом из группы антагонистов фолиевой кислоты, который используется в лечении различных дерматозов.
Большинство дерматологов привыкли его назначать с проведением соответствующего мониторинга, в частности, при лечении псориаза. недавно МТХ был представлен как вариант лечения ГA.
Несмотря на то, что механизм действия метотрексата не совсем ясен, известно, что он ингибирует фермент дигидрофолатредуктазу, что приводит к снижению внутриклеточного концентрации фолата. Это снижение подавляет метаболизм пурина и пиримидина и, следовательно, синтез нуклеиновых кислот, что обеспечивает получение противоопухолевых эффектов при назначении в высоких дозах. Полиглутаматы МТХ также ингибируют AICAR (5-аминоимидазол -4-карбоксамид -рибонуклеотид формилтрансферазу) — фермент, участвующий в синтезе пурина, что в конечном итоге приводит к накоплению аденозина — медиатора многих противовоспалительных эффектов метотрексата.
Аденозин высвобождается во внеклеточное пространство и ингибирует накопление белых клеток крови, приводя к уменьшению ФНО-альфа и синтеза интерферона-гамма, и ингибирует различные моноциты, макрофаги и активность Т-клеток. Это действие может объяснить эффективность применения МТХ при ГА.
Целью исследования стала оценка эффективности и безопасности метотрексата при ГА в виде монотерапии или в комбинации с другими методами лечения, такими как кортикостероиды для перорального приема или внутриочагового введения; и определение терапевтического ответа в зависимости от пола, возраста, типа ГА, длительности заболевания, кумулятивной дозы МТХ и продолжительности терапии. В этом исследовании MTX использовали при тяжелых формах ГА (многоочаговой, универсальной, тотальной и диффузной) с хорошим эффектом (> 50% возобновления роста в 67,7% случаев), с небольшим количеством побочных эффектов. Эти результаты согласуются с результатами предыдущих исследований: Joly (2010) и Droitcourt (2012), которые сообщили об удовлетворительном возобновлении роста у 64% и 70% пациентов, соответственно. В исследовании, проведенном у детей, также не обнаружили каких-либо серьезных побочных эффектов при достижении возобновления роста >50% у пяти из 13 обследованных пациентов.
МТХ в средней дозе 20 мг/неделю, по-видимому, является безопасным и перспективным методом лечения тяжелых форм ГА. Для начала развития положительного эффекта требовалось достижение средней кумулятивной дозы 180 мг, а наилучший эффект наблюдался при общей кумулятивной дозе 1000−1500 мг.
Ограничения данного исследования включают гетерогенность выборки (включались пациенты как из в государственной клиники, так и из частной практики), а также ретроспективный несравнительный характер исследования. Для обоснования применения МТХ в качестве терапии ГА первой линии требуются слепые, рандомизированные исследования бóльшего числа пациентов. В настоящее время изучается применение биомаркеров для мониторинга состояния пациентов, которые могут способствовать долгосрочному использованию МТХ без необходимости проведения инвазивных обследований.

Efficacy and safety of methotrexate in alopecia areata*
Alopecia areata (AA) is a chronic, multifactorial condition of the hair follicles and nails, of unknown etiology, with clear genetic and autoimmune components. It is a common complaint at the dermatologist’s office, accounting for 0.7 to 3.8% of visits. Depending on the number and distribution of lesions and the extent of involvement, it is classified clinically into the following patterns: unifocal, multifocal,
ophiasis, totalis, universalis, sisaipho (or ophiasis inversus), reticular, and diffuse.1 Regarding the natural history of the disease, hair regrowth occurs in 34% to 50% of patients within 1 year, whereas 15% to 25% will progress to AA totals (loss of all scalp hair). There is a direct association
between the severity of AA and the long-term prognosis. Available treatments may induce regrowth, but do not modify the disease course.
Potential autoimmune mechanisms involved in the pathogenesis of AA include sensitization of T lymphocytes, particularly CD8+ T cells, to follicular antigens. Activation of the lymphocytes that compose the perifollicular infiltrate characteristic of AA induces release of several Th1 cytokines — interleukin (IL)-1 alpha, IL-1 beta, and tumor necrosis factor (TNF) alpha — capable of inhibiting hair follicle growth and arresting
hair synthesis, with early termination of anagen.
Several treatment options have been suggested for AA, but there is a paucity of randomized, double-blind, placebo-controlled trials, and, as noted above, no therapy is able to modify the course of the disease.
Methotrexate (MTX) is an immunosuppressant agent of the folic acid antagonist class that is used in the treatment of several dermatoses. Most dermatologists are used to its prescription and monitoring, particularly
in the treatment of psoriasis. MTX was recently introduced as a therapeutic option for AA. Although the mechanism of action of MTX is not entirely understood, it is known to inhibit the enzyme dihydrofolate reductase, which leads to a decrease in intracellular reduced folate concentrations. This decrease inhibits purine and pyrimidine metabolism and, consequently, nucleic acid synthesis, thus resulting in antineoplastic effects when administered at high doses. MTX polyglutamates also inhibit AICAR (5-aminoimidazole-4-carboxamide ribonucleotide formyltransferase), an enzyme involved in purine synthesis, which ultimately leads to a buildup of adenosine, a mediator of many of the anti-inflammatory effects of MTX. Adenosine is released into the extracellular space and, among multiple anti-inflammatory actions, inhibits white blood cell accumulation, leads to a reduction in TNF-α and IFN-γ synthesis, and inhibits a variety of monocyte, macrophage, and T-cell activities. This action might explain the effect of MTX in AA.
The aim of the study to assess the efficacy and safety of MTX in AA,
whether as monotherapy or in combination with other treatment modalities, such as systemic or intralesional corticosteroids; and to assess therapeutic response according to sex, age, pattern of AA, disease duration, cumulative MTX dose, and duration of therapy.
In this study, MTX was used in severe forms of AA (multifocal, universalis, totalis, and diffuse) to good effect (>50% regrowth in 67.7% of cases), with few adverse effects. These results are similar to those reported in prior studies: Joly (2010) and Droitcourt (2012) reported satisfactory regrowth in 64% and 70% of patients respectively. A study conducted in children also found no serious side effects, with >50% regrowth in five of 13 patients assessed.
MTX at a mean dose of 20 mg/week appears to be a safe and promising option for the treatment of severe forms of AA. A mean cumulative dose of 180 mg was required for onset of response, and total cumulative doses of 1000−1500 mg were associated with the best responses.
Limitations of this study include the heterogeneity of the sample (patients recruited from a public hospital and from private practices), as well as the retrospective, non-comparative nature of the study. Larger samples studied in a blind, randomized fashion are required to establish MTX as first-line therapy for AA. The use of biomarkers currently in development for patient monitoring may encourage longterm use of MTX without requiring patients to undergo invasive testing.
